IN PATIENTS WITH OSTEOARTHROSIS

Naphthenic hydrocarbon is extensively used in the rehabilitation of patients with musculoskeletal injuries. Given the complex chemical composition of naphthenic oil petroleum, many researchers rightly believe that a new direction in petroleum therapy should develop along the way of producing a large range of preparations from a naphthenic hydrocarbon, and, if possible, the use of naphthene baths only where strictly indicated. Prospects for the development of petroleum therapy are associated with the further search for highly effective complex effects in diseases of the joints.

For the first time, we have obtained and successfully used naphthenic-dimexide emulsion (NDE) in patients with rheumatoid arthritis (RA) and osteoarthritis (OA). Emulsion has a more pronounced therapeutic effect than naphthenic hydrocarbon and dimexide alone.

In the literature there are many data on ineffective use in the treatment of a single natural factor in the form of monotherapy. Osteoarthrosis is a dystrophic disease, which is based on progressive degeneration of the articular cartilage, up to its loss, accompanied by reporter outgrowths of the marginal surfaces of the joint bones and secondary synovitis. Therefore, to increase the therapeutic effect, combining or combining physical factors is advisable.

To this end, we used NDE in combination with a magnetic field (PeMP) in patients with OA. Studies were conducted in 45 patients with OA. Polyostoarthrosis occurred in 10 patients, gonarthrosis in 30 patients, coxarthrosis in 5 patients. Secondary reactive synovitis of the knee joints was observed in 4 patients. By the method of treatment, the patients were divided into groups II. According to the clinical and functional characteristics of the patients, the groups were comparable. I-gr. - 30 patients received lubrication of naphthenic hydrocarbon-dimexide emulsion on the affected joints, then this field was affected by a magnetic field, the POLUS-1 device was a P-shaped inductor, transversely, in a constant mode, the current shape was sinusoidal, the magnetic intensity was 25 mT, 15 minutes, №10 procedures (NDE + PeMP).

II gr - 15 patients received lubrication on the joints of the naphthenic hydrocarbon with subsequent exposure to this area of the magnetic field, according to the above procedure, 15 minutes, No. 10 procedures (OH + PeMP).

All patients were prescribed a massage and exercise therapy. The following research methods were used: quality of life questionnaire (QOL) of patient, intensity of pain syndrome (according to VAS scale), degree of analgesic effect of studied therapeutic factor (according to RCD formula); X-ray examination of joints. From the biochemical indicators, the malonic dialdehyde (MDA), the activity of catalase and glutathione, the viscosity of the blood, the hematocrit, the degree of erythrocyte aggregation were measured. Immunological parameters were determined by circulating immunocomplexes (CIC), antichondral antibodies (AXA) and nitrosine tetrazolium (NST) test.

When analyzing the results of the therapy in Group I, of 16 points of QoL indices in 10, there was a significant improvement (Q <0.01), a decrease in the QL index (P <0.001) and pain intensity (P <0.05), a degree of analgesic the effect of the studied therapeutic factor was assessed as an average.

Symptoms of synovitis in 2 patients after treatment were not determined.

Biochemical and immunological parameters in this group of patients underwent significant (P <0.01) changes for the better.

On the basis of clinical and functional indices in patients with OA who received NDE + PeMP, an improvement was found in 76.7% of patients, without effect in 23.3%.

In the II group of patients who received lubrication OH + PeMP after treatment from 16 points defining QL in 6, significant improvement was found (P <0.05).

The QoL index, the intensity of pain in this group of patients underwent changes in the lower side (P <0.05), the degree of analgesic effect of the therapeutic factor was estimated as an average. Symptoms of synovitis from 3 patients in 2 patients after treatment were not determined.

From biochemical studies against the background of a significant increase in catalase (P <0.05), there was a tendency to increase antioxidant activity of blood, glutathione and decrease of malonic dialdehyde (P <0.05). In this case, the rheological properties of the blood (P <0.05), as well as the positive dynamics of immunological parameters, are improved.

On the basis of clinical and functional indices in patients with OA who received OH + PeMP, an improvement was found in 60% of patients, with no effect in 40%.
